Live independently in your own home as you age - we want to help you achieve that goal. Through CHT Home Care, we offer an extensive range of in-home services that are designed to meet your needs. CHT are a local, well regarded provider with a local office and staff - we can assist you through the process. Each client has an experienced Registered Nurse as their Case Manager - regardless of package level, each Client can receive guidance and support. CHT offer our Home Care clients priority access to our Residential Aged Care homes at either Hawthorn Village or Snug Village for your peace of mind should the need arise.
